Waiver

General Statement of Program Objectives and Procedures
I understand that this physical fitness program includes exercises to build the cardiovascular system (heart and lungs), the musculoskeletal system (muscle endurance, strength, and flexibility), and to improve body composition (decrease/increase muscle mass, decrease body fat, and increase bone density). Exercise may include aerobic activities (treadmill, walking, running, bicycle, rowing, group exercise, swimming, and other aerobic activities), callisthenic exercise, and weight lifting to improve movement and functionality. 


Description of Potential Risk
I understand that the reaction of the heart, lungs, and blood vessels cannot always be predicted with accuracy.   I know there are risks of certain abnormal changes occurring during and following exercise which may include abnormalities of blood pressure or heart attacks. Use of weights, lifting techniques, and calisthenics may lead to musculoskeletal strains, sprains, pain and injury if adequate warm up, gradual progression, and safety procedures are not followed. I understand that personal trainer Gymnos Fitness, LLC shall not be liable for any damages arising from personal injuries sustained while and during the personal training program and using the exercise equipment during training does so at his/her own risk.  


Client assumes full responsibility for any injuries/damages which may occur during training. I hereby fully and forever release and discharge Gymnos Fitness, its assigns, and agents from all claims, demands, damages, rights of action, present and future. 


I understand and warrant, release and agree that I am in good physical condition and that I have no disabilities, impairment or ailments preventing me from engaging in active/passive exercise that will be detrimental to the heart, safety, comfort, or physical condition if I engage/participate (other than those items fully discussed on Health History Questioner).


I state that I have had a recent physical check-up and have my personal physician’s permission to engage in aerobic/anaerobic conditioning. 


Description of Potential Benefits
I understand that a program of regular exercise for the heart, lungs, muscles, and joints has many benefits associated with it. These may include in a decrease in body fat, increase muscle mass, improvement in blood fats, blood pressure, resting heart rate, mobility, improved physiological function, and decrease in heart disease.
